Criteria for reporting progress and changes

 

The investigators shall provide the KNH/UON-ERRC with progressive reports on a yearly basis prior to renewing the proposal.  The report shall have the following information:

 

1.                   Title of the project

2.                   Names, addresses  and  qualification of the investigators and changes if any

3.                   Number of subjects expected to be recruited

4.                   Number recruited in the previous request for renew

5.                   Number recruited during the previous year

6.                   Number of protocol violations

7.                   Number of  SAE

8.                   Problems encountered

9.                   Changes in funding
